What to Expect Before, During, and After Your Initial Appointment

Before your Initial Appointment

We want to help streamline your first appointment so that you don’t have to spend time filling out patient forms at our office. To accomplish this, we have made all required new patient forms available to complete upon your registration with our practice & patient portal. The forms are also available online for you to print and complete before your first appointment.

  • Please complete your patient portal registration within 24 hours or send these completed forms to our office when scheduling your first appointment.
  • You may email the completed forms to the appropriate office before your appointment using the following email addresses:

Please review the new patient no show policy and fees before your appointment and call one of our offices if you have any questions. Remember that a $50 deposit is due before your appointment can be scheduled.

Your initial visit to one of our Physician’s Weight Control and Wellness Center offices is important, as we will discuss your medical history and lifestyle to develop a customized plan for weight loss. If you have children, we ask that you arrange child care during your appointment to keep the focus on you and your weight loss goals.

On the Day of your Initial Appointment

Please bring your driver’s license or another form of picture identification to your first appointment for our records.

Bring payment (debit, credit, money order, or cash) for the new patient fee, due on the day of visit.

When you visit our office for the first time, you can expect the following:

  • Basic tests to create your unique wellness profile
    • Blood Testing – we will perform a complete blood panel to provide an overview of your body’s unique chemistry. This series of blood tests will check your glucose, cholesterol, triglycerides, thyroid levels, and other levels. We will go over your results on your second visit.
    • Electrocardiogram (EKG) – we will administer an EKG to measure your heart’s health. These results will also be discussed during your visit.
    • If these tests show any results that are dangerously out of range, we will let you know before your next scheduled visit.
  • Plan of treatment
    • Our doctor will recommend a plan of treatment to begin prior to your next scheduled appointment. This play may include any or all of the following:
      • Supplements
      • Lipotropic vitamin B injections (optional)
      • Exercise programs
      • Meal plans
      • Appetite suppressants or other medications (when appropriate)

After your Initial Appointment

After your initial visit, you will be considered an established patient. We will schedule your next appointment, typically four weeks from your initial visit.

As an established patient, you will be subject to our established patient no-show policies and fees for all appointments after your first visit.

If there are any abnormal results from your initial blood testing or EKG, our office will call you to discuss before your next appointment. 

Other Useful Information

Insurance

Historically, insurance companies have not provided coverage for weight loss programs. However, some companies are becoming more receptive to the idea of covering treatment for obesity.

Please note that we do not have the resources in our office to file health insurance claims or to handle other insurance-related issues on your behalf. As such, there will be an additional charge for any forms that you or your insurance company asks our office to complete and return.

While we do not file health insurance claims in our office, we will gladly give you all the information you need to file your own claim for reimbursement in the form of our comprehensive bill. We can provide this bill on the day of your appointment should you request it.

Payment

We accept cash, money orders, debit cards, and the following credit cards: Mastercard, Visa, Discover, and American Express. We do not accept checks as payment.

Hours

Because our providers rotate between the offices, our hours vary greatly from office to office. Please call for appointments at the office of your choice.

Appointments

Appointments are required to visit our office for treatment. Please call your preferred office location to schedule an appointment, preferably several weeks ahead of your desired date as our providers’ schedules fill up fast.

We do not schedule, reschedule or cancel appointments through this website or email.  Please call your preferred office to schedule, reschedule or cancel.

New Patient Forms

In order to save time filling out forms when you come to your first appointment we request that all new patients print and read all the documents and complete the forms listed here. Arlington patients, you will need to bring these completed forms to your first appointment. Waco patients, please have these forms filled out at the time you call to make your first appointment.
