- Complete the online intake form on this page. Information submitted here is encrypted and HIPAA-compliant.
- Call to schedule your first visit. A $50 deposit is required before we can schedule; it applies toward your new patient fee when you attend.
- Finish your paperwork — register on the patient portal within 24 hours, or download and return forms from our Download Center (email Arlington or Waco office).
- Prepare for labs — arrive fasting at least 8 hours (water and black coffee are allowed). Bring photo ID and payment for the visit balance.
- Come to your first visit — consultation, blood panel, and baseline EKG are part of the $295 new patient visit (same for Arlington and Waco).

Scheduling deposit
$50
Due before we can schedule your initial appointment. Non-refundable if you miss or fail to reschedule in time; applied to your visit fee when you attend.
First visit
$295
Same for all offices. Includes consultation, blood testing, EKG, and treatment elements administered that day. After the $50 deposit, $245 is due at the visit.
